chore: untrack ralph-tui state, ignore packaging output

.ralph-tui/ is runtime state from the Ralph TUI agent-loop runner: iteration
counters, run logs and a stale lock file from an interrupted January session.
It was only tracked because that tool runs with autoCommit. Untracked and
ignored, along with dist/ (Debian packaging output).
