docs: record the LMDB per-process POSIX lock hazard and the count surfaces

Two things worth not relearning: closing a second MDB_env on an
already-open path drops the whole process's POSIX locks (90s read outage on
the first 2.4.4 boot), and 'smartbotic-db-cli count' goes through
GetCollectionInfo rather than the Count RPC.
